The fundamental rule of thumb depends almost entirely on the female partner's age.
1. Under Age 35: You should seek an evaluation if you have been having regular, unprotected sex for 12 months without conceiving.
2. Age 35 to 39: The window shortens. Seek evaluation after 6 months of trying. This is because egg quality and quantity begin a more rapid decline after 35, and waiting a full year wastes critical time.
3. Age 40 and Over: Seek an evaluation immediately when you decide to start trying. A specialist can run baseline tests to ensure your efforts are worthwhile and not hindered by an easily identifiable blocker.
When to Ignore the Guidelines and Go IMMEDIATELY:
You should not wait 6 or 12 months if there are known underlying conditions that impact fertility. Book a consultation right away if:
For Women:
- Your periods are highly irregular, absent, or extremely painful (suspected PCOS or Endometriosis).
- You have a history of pelvic infections (PID) or ruptured appendix, which could signal blocked tubes.
- You have had two or more consecutive miscarriages.
- You have a family history of early menopause.
For Men:
- There is difficulty achieving an erection or ejaculating.
- A history of testicular trauma, surgery, or childhood cancer treatments.
- Previous testosterone replacement therapy (TRT) use (which halts sperm production).
