IUI vs IVF — How to Decide Which to Try First

IUI is appropriate when:
- Both fallopian tubes are open
- Sperm count and motility are mild-to-moderately reduced (count >5 million motile)
- Ovulation can be stimulated or timed effectively
- The woman is under 35 with good ovarian reserve
- The diagnosis is unexplained infertility and you have time to try this gentler step
- Cost is a significant consideration (IUI costs ₹8,000–₹20,000 vs ₹1.2L–₹2.5L for IVF)
IVF is recommended directly when:
- Both fallopian tubes are blocked
- Severe male factor infertility (requires ICSI)
- Female age is 38 or older
- Failed 3–4 IUI cycles
- Advanced endometriosis (Stage III–IV)
- Very low AMH or poor ovarian reserve
- PGT (genetic testing of embryos) is needed
The important comparison:
- IUI success rate: 15–22% per cycle
- IVF success rate: 55–70% per cycle (under 35)
Skipping IUI to go straight to IVF is sometimes the right decision — especially for older women where time is critical. Spending 4–6 months on IUI cycles that have little chance of success can mean a critical reduction in IVF success due to age.
