Early Signs of Pregnancy After IVF Transfer — What to Look For

The 'two-week wait' between embryo transfer and the pregnancy test is often the most anxiety-inducing part of IVF. You analyse every twinge. But what are the real signs of pregnancy, and what is just a side effect of medication?

The truth about symptoms: The most important thing to know is that many early pregnancy symptoms are identical to the side effects of the progesterone and estrogen you are taking for the IVF cycle. Having symptoms does not strictly guarantee pregnancy, and having NO symptoms does not mean the cycle failed. Common signs (which could be pregnancy OR medication): 1. Spotting or light bleeding: Often called 'implantation bleeding', this can occur 5-7 days after transfer. It is usually light pink or brown and very brief. However, spotting can also just be a side effect of progesterone suppositories or the transfer procedure itself. 2. Cramping: Mild cramping is very common as the uterus prepares for implantation, but also common from progesterone. Severe cramping should be reported to your doctor. 3. Breast tenderness: Swollen, heavy, or tender breasts are classic early pregnancy signs, but also very common from hormone medications. 4. Fatigue: Profound tiredness is a hallmark of early pregnancy (due to rising progesterone levels), but again, you are taking supplemental progesterone. 5. Nausea: True 'morning sickness' usually starts closer to 6 weeks, but some women report mild nausea or food aversions earlier. 6. Frequent urination: Hormonal changes and increased blood volume can make you need the bathroom more often. 7. Missed period: If you are taking progesterone, it will usually prevent your period from starting, regardless of whether you are pregnant. When to test: We strongly advise waiting for the scheduled beta-hCG blood test (usually 10-14 days post-transfer). Home urine tests can give false positives (from an HCG trigger shot) or false negatives (if tested too early). When to contact the clinic immediately: - Heavy bleeding (like a strong period) - Severe abdominal or pelvic pain - Fever or flu-like symptoms
