The first 48–72 hours: Repeat beta-hCG A single positive beta-hCG is not enough — we always repeat the test 48–72 hours later. The level should roughly double in this time. A rising beta confirms a developing pregnancy. Levels that plateau or fall may indicate a chemical pregnancy or early loss — devastating but important to know quickly. Week 5–6: First ultrasound We look for the gestational sac and, by 6 weeks, a fetal heartbeat. This first heartbeat scan after IVF is one of the most emotional moments for our patients — and for us. Continuing medications: Most IVF pregnancies require progesterone supplementation (pessaries or injections) until 10–12 weeks of pregnancy. Do not stop without Dr. Ankita's guidance — the placenta takes over progesterone production gradually. Stopping too early risks early pregnancy loss. After the first trimester: - Most IVF pregnancies following this are managed like any other pregnancy - Twin pregnancies (more common with IVF) require closer monitoring throughout - Women with underlying conditions (PCOS, thyroid, fibroids) need ongoing surveillance Handling anxiety: Postpartum anxiety and hypervigilance about baby movements, spotting, and symptoms are extremely common after a difficult fertility journey.
