Azoospermia Explained — Zero Sperm in the Ejaculate: What It Means and What You Can Do

Azoospermia affects approximately 1 in 100 men and about 10–15% of all infertile men. There are two types — and the type determines the treatment. Type 1: Obstructive Azoospermia (OA) Sperm are being produced normally in the testes but cannot exit due to a blockage — in the epididymis, vas deferens, or ejaculatory ducts. Causes: Vasectomy, previous epididymal infection, congenital bilateral absence of vas deferens (CBAVD, linked to CFTR gene), prior hernia surgery that accidentally damaged the vas. Sperm retrieval success: Very high (>90%) with PESA or TESA. Quality of retrieved sperm is excellent. Type 2: Non-Obstructive Azoospermia (NOA) The testes are not producing enough sperm — due to testicular failure, hormonal disruption, or genetic abnormality. Causes: Klinefelter syndrome (47XXY), Y-chromosome microdeletions, Sertoli cell only syndrome, previous chemotherapy, mumps orchitis, undescended testes. Sperm retrieval: Possible in 40–60% of NOA cases via micro-TESE — a microsurgical technique that searches the testicle for small pockets of active sperm production. How is it diagnosed? Repeat semen analysis (confirm) → FSH/LH/testosterone levels → Karyotype + Y-microdeletion → Testicular ultrasound → Testicular biopsy in selected cases. FSH interpretation: - Low/normal FSH = likely obstructive (good prognosis) - High FSH = testicular impairment (non-obstructive; prognosis variable) What if no sperm are found? Donor sperm via IUI or IVF remains an option — allowing the female partner to be the biological mother. Dr. Chhabi Ghosh discusses all options with complete transparency and compassion.
