Tubal Ligation Reversal vs IVF: Growing Your Family After 'Tied Tubes'
Life changes, new relationships form, and many women who previously had a tubal ligation (their 'tubes tied') find themselves wanting another child. The great news is that you have excellent options. But the decision of how to proceed—surgical reversal or IVF—requires careful thought.

Option 1: Tubal Ligation Reversal Surgery
This is a microsurgical procedure where a surgeon reconnects the blocked segments of the fallopian tubes, allowing sperm and egg to meet naturally again.
When is Reversal a Good Idea?
- You are under 35 with excellent ovarian reserve (AMH).
- Your male partner has an excellent standard semen analysis.
- Only a small portion of the tube was removed initially (requires getting the operative report from the original surgery).
- You want the possibility to conceive naturally more than once in the future.
The Downsides of Reversal:
- Success is not guaranteed (tubes may scar shut again).
- High risk of Ectopic Pregnancy.
- Takes a long time (recovery from surgery, plus up to 1-2 years of trying naturally).
Option 2: In Vitro Fertilization (IVF)
IVF bypasses the fallopian tubes entirely. Eggs are retrieved directly from the ovaries, fertilized in the lab, and the embryo is placed directly into the uterus.
When is IVF the Better Choice?
- You are 37 or older. Time is critical for egg quality, and waiting a year after surgery to 'see if it works' is too risky.
- There is male factor infertility (low sperm count). Reversing the tubes won't help if the sperm cannot reach the egg.
- You want to conceive as quickly as possible.
- Parts of the tubes (like the fimbriae) were removed or burned (cauterized) making reversal nearly impossible.
The Verdict on Cost and Success:
Initially, a reversal surgery might seem cheaper. However, if the reversal fails after a year, you will still need to pay for IVF. IVF offers a significantly faster time-to-pregnancy and the highest per-cycle success rates, especially when age is a factor.
