Male Fertility Testing Explained: It Is More Than Just 'Sperm Count'
When couples face fertility struggles, the focus often immediately shifts to the woman. However, male factor infertility contributes to 40-50% of cases. The cornerstone of evaluation is the semen analysis, but 'count' is only one small part of the story.

What a Semen Analysis Really Looks At:
A standard semen analysis (following WHO 2021 criteria) evaluates the sample across several dimensions:
1. Volume: How much semen is produced. Low volume suggests blockage or retrograde ejaculation.
2. Concentration (Count): How many sperm are in each milliliter. Normal is >16 million/mL.
3. Motility: How many sperm are moving (at least 42%) and how well they are moving (progressive motility — swimming forward strongly vs swimming in circles).
4. Morphology: The shape of the sperm. Normal sperm have an oval head, a connecting piece, and a long tail. Even in highly fertile men, 96% of sperm are abnormally shaped. Normal morphology is >4%.
5. Vitality: What percentage of non-moving sperm are actually alive.
6. Liquefaction time: Semen is initially thick and should liquefy within 30-60 minutes to allow sperm to swim.
The Missing Piece: DNA Fragmentation Index (DFI)
A standard semen analysis checks what sperm look like and how they move. It DOES NOT check the genetic cargo inside. Sperm DNA Fragmentation refers to damage or breaks in the DNA strands within the sperm head. A man can have a totally 'normal' semen analysis but very high DFI, which causes IVF failure and recurrent miscarriage. This requires a separate, specialised test.
Preparing for the Test:
For accurate results, abstain from ejaculation for 2 to 5 days before the test. Do not abstain for longer than 7 days, as sperm quality begins to degrade.
What if the results are abnormal?
A single poor semen analysis is not a diagnosis. Sperm fluctuate dramatically based on illness, stress, and heat exposure. It takes ~75 days for new sperm to form. We always recommend repeating the test after 4-6 weeks before finalizing a diagnosis or treatment plan.
