Endometrial Thickness: Cultivating the Perfect Soil for Your Embryo
In IVF, we spend tremendous effort cultivating the perfect 'seed' (the embryo). But the seed is only half the equation; the 'soil' must be ready to receive it. In fertility medicine, the soil is the endometrium—the lining of your uterus.

The Optimal Lining
During an ultrasound monitoring appointment before an embryo transfer, your doctor is looking for two key features in the endometrium:
1. Thickness: The ideal measurement is between 8mm and 12mm.
2. Pattern: We want to see a 'Triple-Line' or 'Trilaminar' pattern. This indicates the lining has responded perfectly to estrogen and is plush and hospitable.
What if the lining is too thin (Under 7mm)?
A lining thinner than 7mm is associated with significantly lower implantation rates and higher rates of early miscarriage. If a patient does not reach 7mm, Dr. Ankita will often make the difficult but medically sound decision to cancel the transfer cycle. It is better to wait another month and adjust medications than to waste a precious embryo on unready soil.
Causes of a Thin Lining:
- Poor blood flow to the uterus
- Previous uterine trauma (repeated D&C procedures, Asherman's syndrome, scarring)
- Prolonged use of certain oral contraceptives or Clomid in the past
- Chronic endometritis (an underlying low-grade infection)
How do we improve the lining?
If you struggle with a thin lining, your protocol for the next month needs adjustment. Strategies include:
- Extended Estrogen: Providing more time and higher doses of estrogen (oral, vaginal, or transdermal patches).
- Improving Blood Flow: Using low-dose aspirin, Vitamin E, or L-arginine supplements. Acupuncture has also shown efficacy in improving pelvic blood flow.
- Hysteroscopy: Looking inside the cavity to clear any tiny adhesions or scar tissue.
- PRP (Platelet-Rich Plasma): In stubborn cases, injecting your own concentrated blood plasma into the cavity can stimulate lining regeneration.
